Resolution

The documentation for the migration toolkit does not specify the rights that are needed for the "Username"  field on the Exchange configuration screen. So the customer assumed they should use a user with Domain Administration rights. They clicked the "TEST" button and the test passed. So they figured that they had chosen an appropriate user.

After the failure and error noted above, the customer then chose an user who was in the "Exchange Organization Management" role group. Using that user resolved this problem.
